The Keizer Chamber is very excited work with Olson Insurance to deliver an opportunity to provide affordable, relevant, and tailored healthcare to employers through an affiliate program with the Bend Chamber of Commerce.  Providence Health Plans and Moda Dental are proud to partner with the Chamber to provide network and plan options that serve the covered businesses and their members’ needs. 

We will be having a Lunch and Learn with Olson Insurance on September 25th, 2024. You can register on our website here. It will be $15 for lunch or you can RSVP no lunch, just attending.
 
To take advantage of this offer, and to receive an estimate for your company, the following must apply:

1. You are a business that is headquartered in the State of Oregon
2. You are a member of the Keizer Chamber
3. You have at least two W2 employees eligible and enrolling

-Premier, Core and Base plans with a wide range of deductibles
-Common deductibles for most Premier Plans
-HSA Qualified plans with an embedded deductible
-Two pharmacy plan options for Premier plans
-Many preventive medications with a $0 copay
-90-day supply of maintenance medications for two copayments
-Optional vision benefit that can be added
-$1,000 accident benefit on all non-HSA plans
-AD&D benefit of $25,000 for employees enrolled in the medical plan through USAble Life
-Four visit Employee Assistance Program
